Situated in Zone 5, Sutton is less than 30 minutes by train from central London, and south and south west London are easily accessible by public transport. There are 10 train stations which serve key neighbourhoods within the borough providing direct connections to London Victoria, London Bridge, London Blackfriars and St Pancras International. Sutton also has direct rail connections with Epsom, Dorking, Guildford and Horsham, as well as Wimbledon for an interchange with the London Underground and West Croydon for the London Overground. There’s also an extensive road network which provides close and easy access to the M25 and M23.

As a Specialist Support Worker in the Children and Young People's Disability Service, you will provide a range of support and interventions designed to meet family needs in a flexible and responsive way. Guided by the principles of Restorative Practices you will work in family homes and communities, completing direct work with children, young people and their families.

You will:

- Explore & create bespoke Short Breaks support packages with children & their families

- Assess child and family need through the Early Help Assessment Tool

- Carry out clear Transition Assessment & Preparation into Adulthood

-  Provide direct and indirect specialist support work which will include support to access services, developing parenting strategies and behaviour management techniques 

- Hold Child In Need Overnight Provision Short Breaks cases and regularly hold CIN Review meetings to ensure the right support is in place for both the child and their family.

We are a specialist service that have a strong team consisting of Specialist Support Workers, Social Workers, Occupational Therapists and Finance that all work towards supporting children, young people and their families with managing their additional needs.   Our aim is to help support families develop their long term resilience in managing their child or young person's needs. This may be through direct work with parent or child, sign-posting to universal or specialist services and assessing need to support through the form of a short breaks package. We have developed strong protocols and processes to support this process and continue to work on Transition's development to ensure a smooth transition for young people into adulthood. 

We welcome applications from individuals who have experience of whole family working and  direct work with children and young people. You will be skilled in undertaking Early Help Assessments, identifying needs and having a solution focused attitude, identifying key outcomes.  Excellent communication and relationship building skills will enable you to provide advice and support to the multi-agency partnership to ensure families are supported. Experience of working with children with disabilities would be an advantage.
